Medical Home Huddle Improves Ambulatory Care

Every morning, the SBH Ambulatory Care Center medical floor initiates a huddle to start the day. The entire treatment team meets at the nurses’ station for 10 minutes to discuss the day’s plans. The multidisciplinary team includes nurses, attending physicians and residents, medical assistants, care navigators, registrars, social works, and medical students. Everyone is encouraged to participate. One of the most important elements of the huddle is a discussion of the high-risk or new patients scheduled for the day. The goal is to make sure everyone is well-prepared in advance and knows what to expect. Having a well-informed team improves quality care.
