    Dr. Rampersaud completed her PharmD at Touro College of Pharmacy, followed by her PGY-1 Pharmacy Residency at Brookdale Hospital Medical Center in Brooklyn, NY. Presently, at SBH Health System in Bronx, NY, Dr. Rampersaud coordinates and oversees all aspects of Care Transitions involving Medication Reconciliation, Patient Education and SBH’s Meds-to-Beds Program.

    In her role as Internal Medicine Clinical Coordinator she participates in daily interdisciplinary rounds with the internal medicine teams and fulfills her role as preceptor for pharmacy residents and students in Internal Medicine and Transitions of Care. She frequently provides educational trainings and in-services for the house staff at SBH and coordinates various community outreach activities. Her areas of interest are Internal Medicine, Cardiology and Transitions of Care.
