The Senior Health Center addresses the health care needs of older adults. We provide primary care, consultations, and co-managed care for our senior patients. Our team consists of a board certified geriatrician, a geriatric nurse practitioner, a nurse case manager, and patient navigators take special care to check on all aspects of healthy aging. Our mission is to keep our patients’ lives vibrant, independent, and satisfying. We help our patients and those that might be assisting them at home, family and friends, to stay active and enjoy their lives.
We screen and treat for conditions common to seniors; memory loss, coping with multiple health conditions, managing medications, falls and gait and balance difficulties, incontinence, frailty and weakness, and late-life depression.
Our Center hosts educational sessions for caregivers including training and support from the New York Chapter of the Alzheimer’s Association.
We also have multiple services on-site including a pharmacist to assist in complicated medication management, podiatry and foot care, diabetes care, and social work counseling.
We go to great lengths to present a welcoming location that our seniors can easily access and get the help they need in order to avoid unnecessary emergencies and hospital visits.
