We conduct an intensive assessment of your patients’ medical, psychosocial and functional capabilities and coordinate with you to provide specialized and comprehensive care and prevent the need for hospital care whenever possible. If an inpatient stay is necessary, we offer private and semi-private rooms designed exclusively for older patients.
