WIC Program

WIC (Women, Infants, and Children) is a federal assistance program providing nutritional support to low-income pregnant women, breastfeeding mothers, infants, and young children up to age five. The program offers access to healthy foods, nutrition education, breastfeeding support, and referrals to healthcare and social services, helping to ensure a healthy start during critical stages of life.

What is WIC?

The SBH Health System WIC Program is a federal and state-funded nutrition education and supplemental food program. The WIC Program provides nutrition, health, fitness, and breastfeeding information as well as monthly nutritional tailored food packages for eligible participants.

 

Pregnant, breastfeeding and postpartum women and children up to age 5 who meet the medical, nutritional, residential and income requirements are eligible.

You will receive information and classes on nutrition, wellness, fitness, breastfeeding, farmers market checks (seasonally), and how to access other services our WIC program provides.

WIC Forms

Our WIC Forms webpage offers a convenient way to access all the necessary forms, from application materials to income guidelines, to help you enroll in the program and receive the support you need. Whether you’re applying for the first time or updating your information, SBH Health System is here to ensure a seamless process for you and your family.

The WIC-Medical Documentation form may be used to refer patients to the WIC Program and to communicate changes in patient health information. The information provided on this form will be used by a WIC nutritionist to determine nutrition care and provide nutrition counseling.

Use the WIC Special Formula form below to request exempt formulas, WIC-Eligible Nutritional’s, standard formulas for infants unable to tolerate solid foods, and supplemental foods for patients with qualifying medical conditions. If you have questions or need additional clarification, please contact the SBH WIC Offices.

Breastfeeding Peer Counselor Program

The position of WIC Breastfeeding Peer Counselor is a full-time role where bilingual candidates (English/Spanish) are preferred. Your primary responsibility will be to provide essential information about breastfeeding to WIC participants during pregnancy and after their baby is born. You will also encourage participants to breastfeed and assist mothers in finding help if any challenges arise. A high school diploma or equivalent is required, along with personal experience of having breastfed at least one baby for a minimum of six months, although current breastfeeding is not necessary. WIC Farmer’s Market 2024 Season

The 2024 Farmers’ Market season kicks off on June 1st, bringing fresh, locally grown fruits and vegetables to WIC participants through the Farmers’ Market Nutrition Program (FMNP). This program offers WIC families the opportunity to use their FMNP checks to purchase nutritious produce directly from local farmers, supporting both health and community. The market season runs through November 30th, so be sure to visit your local farmers’ market and take advantage of the abundance of fresh options. What is a farmer’s market voucher?

A farmer’s market voucher is a type of coupon or check provided through programs like the Farmers’ Market Nutrition Program (FMNP) or similar initiatives. These vouchers are given to eligible individuals, such as WIC participants, to purchase fresh, locally grown fruits, vegetables, and herbs directly from farmers’ markets. The aim is to support healthy eating while also boosting local agriculture.

LINC Baby Talk

Is your baby starting to learn words or identify objects? This series of workshops is an introduction to how to develop early language skills with your child. We’ll offer tips you can add to your daily routine to take your child’s vocabulary to the next level. Together, we explore how activities like singing, playing, talking, and reading with your child can support their lifelong learning.
