We proudly offer a comprehensive range of services tailored to our senior citizens, recognizing and addressing their diverse health needs with respect, sensitivity, and compassion. Understanding that seniors often face multiple chronic conditions, we are dedicated to delivering exceptional care through the expertise and commitment of our skilled health professionals
Dedicated to Your Well-being in Every Stage of Life
Our Senior need special care because aging brings unique health challenges, such as chronic conditions and mobility issues. At SBH, our specialized care ensures that seniors receive personalized attention, tailored treatments, and compassionate support, promoting a higher quality of life and helping them maintain their independence for as long as possible.
Senior Health Center
Our services include case management, family and patient counseling to enhance independence and improve quality of life at home.
We also organize educational events, often in collaboration with the Alzheimer’s Association, to support families managing the challenges of an aging relative.
Our clinic provides full access to a range of services Monday through Friday, including pharmacology, diabetes management, podiatry, and dental screening. Seniors can continue seeing their primary doctor in the community or clinic while benefiting from the additional support of our specialized team.
Acute Care for the Elderly (ACE unit)
With multiple decades of experience in managing the inpatient care of seniors, our staff is dedicated to promoting early ambulation, orientation, and nutrition during recovery from serious illnesses.
Our approach is designed to improve patient outcomes by reducing episodes of confusion, enhancing independence, and ensuring better physical condition upon discharge.
Additionally, we focus on facilitating safer transitions to home, ensuring that seniors continue to receive the support they need beyond their hospital stay.
Geriatrics Emergency Department Program
With another generous grant from The Fan Fox and Leslie R. Samuels Foundation, Inc., in 2013, we have piloted a program which features a geriatrics assessment team that supports emergency physicians in caring for seniors in our ED. This collaborative effort effectively addresses the complex health issues of elderly patients, which can be challenging in the high-pressure environment of the emergency department.
The program has achieved significant results, including a reduction in admission rates for seniors, increased referrals to essential medical and social services, and a notable decrease in the length of stay for seniors who are evaluated and discharged.
