Our primary care providers walk with you on the health journey. Whether it’s helping you manage a lifelong condition to connecting you with a specialist if you need one, we partner with you every step of the way.

We provide annual wellness checks and recommended screenings when you need them. Whether you need a blood test to check your cholesterol, prevent diabetes or an osteoporosis test from a specialist, we’re your first stop for whole-person health.

At SBH Health System, we’re guided by our commitment to helping you live your healthiest life. One of the ways we do this is through prevention. By meeting with you regularly and getting to know you and your health story, we can recommend immunizations and screenings you may need to stay on top of your wellness — and prevent health conditions from developing. We offer tests and screenings for a wide range of common conditions and, if your needs require more focused care, we can connect you with a specialist in the SBH Health System. For example, we routinely refer many patients to our OB-GYN team for help with fertility needs or pregnancy care, with gynecologic needs, or for breast cancer and osteoporosis screenings.

We also provide programs to help you choose healthier lifestyles, including those that help you quit smoking or lose weight. But we don’t just tell you what you need to do, we lead by example — often sharing personal stories of what worked for us in our own lives.
